有时候我想随意: 不及格的考试

我的临床考试从来没有不及格过，

今天却面对了人生第一次临床考试失败。

话说，儿科出现了一个严格的女教授。每个同学都不希望自己被她挑中。

倒霉的是我们一组4名学生，就这样被她选中了，当了我们的主考官。

每位同学各自有10分钟的时间，以一对一的方式进行临床对话考试。

女教授带我到一位小女孩的房间，我一看：小女孩呼吸困难，戴着氧气，趴在桌上，看起来很辛苦。

女教授说: Please do the respiratory examination.（请检查呼吸系统）

我：好，那我先开始形容这位病人。Bla bla bla...（说着说那...）

女教授：Don't waste your time, quickly examine the posterior chest!Faster!（不要浪费时间，快点去检查肺的后面部分就可以了，快点！）

我：ooo...okokok...(哦...好好好。)

检查，听呼吸，敲一敲，有发现！两边的呼吸是不一样的，声音也不一样！）

我：Prof, on percussion, there is dullness over whole left lung, but the right lung is normal, while on auscultation, the left lung has reduced air entry compare to right lung. There is no crepitation and rhonchi heard...（我向教授解释我得到的结果,不好意思，这句我懒得翻译）

教授：What is your differential diagnosis?(你的诊断是什么)

我: Based on finding of dullness on percussion and reduced air entry, my differential diagnosis would be pleural effusion, and consolidation of lung.（根据上述的症状，我的初步诊断是胸腔积水或是肺炎）

教授: Any other diagnosis?(还有其他诊断？)

我：Ya,it can be lung metastatis.（癌细胞扩散至肺）

教授：Any other?（还有呢？）

我：...(沉默20秒).

教授：Lung collapse(肺萎陷！) OK,tell me how did you differentiate lung collapse and pleural effusion.（告诉我如何分辨肺萎陷和胸腔积水）

我：Tracheal deviation in lung collapse,occasional crepitation in pleural effusion（肺萎陷会有气管倾斜，胸腔积水会听到crepitation-一种发自肺泡的噼啪声）

教授：What other causes of unilateral pleural effusion?（有什么东西可以造成单边的肺积水？）

我：Heart failure（心力衰歇）...？

教授：Ok,done.I fail you.You can't even tell me the differences between lung collapse and pleural effusion.And this patient has cyanosis,but you did not pick up the sign.(好，我不让你及格，你连要怎么分辨费萎陷和胸腔积水都不会，还有，这病人脸色这么蓝，你却没看出来)
